Privacy policy, GDPR, and cookies

This Privacy Policy describes how Sebastian Piechocki („therapist”, „data controller”) collects, processes, and protects the personal data of his clients and website users in accordance with the provisions of the GDPR.

Types and methods of data collection

1. Depending on the method of contact and the type of service, the following data may be collected:
   • first and last name,
   • email address,
   • phone number,
   • dates and times of appointments,
   • payment status,
   • information about the number of therapy sessions,
   • data regarding website visits (e.g. IP address, device, server logs).
2. Clinical data is collected exclusively based on information obtained during therapy sessions and is recorded in the form of notes in a manner that prevents the identification of the individual. This data may include information regarding, for example, mental well-being, a description of difficulties, and therapy goals.
3. The Twój Psycholog or Znany Lekarz appointment scheduling system („data processor”) collects data such as: first name, last name, email address, phone number, selected appointment date, and payment status.

Purposes of data processing

1. Personal data collected during the appointment booking process is processed solely for the purposes of booking, contacting the client, and organizing and billing for therapy sessions.
2. Data collected during the psychotherapy process is used to conduct the therapy effectively.
3. Data collected via cookies (upon consent) is used to improve the functionality of the website.

Data storage and security

1. Data related to appointment bookings made through Twój Psycholog or Znany Lekarz is stored in these systems in accordance with applicable data protection regulations (GDPR).
2. Data stored outside the systems listed in point 1 is located on devices secured with passwords and encryption.
3. Notes taken during therapy sessions are kept in a manner that prevents third parties from identifying the Client.

Data sharing

Personal and clinical data may be disclosed to third parties only when: required by law (e.g. a court order); there is a risk of serious harm or a threat to life; or when discussed as part of supervision (always in a manner that prevents identification of the individual).

Right of access to data

You have the right at any time to access your data, have it deleted, restrict its processing, withdraw your consent to processing, and object to processing.

Cookies

1. The website uses cookies to ensure the proper functioning of the site, as well as (only after obtaining the user’s consent) for analytical purposes.
2. Types of cookies:
   • essential cookies – ensure the proper functioning of the website and do not require the user’s consent;
   • analytical and functional cookies – used to analyze website traffic (visit duration, pages visited, device, and operating system) and to remember user settings (if applicable); they are used only with the user’s consent.
3. Consent to cookies is given by the user upon entering the website and is retained for subsequent visits, provided that the browser settings allow it.
4. You may withdraw your consent at any time by changing your web browser settings.

Other formal matters

1. This Privacy Policy is effective in the form in which it is published on this website.
2. Any version of the Privacy Policy newly published on the website is the current and effective version and supersedes all previous versions.
